Document Required -
Nonimmigrant Visa Application (DS-156).
Supplemental Nonimmigrant Visa Application (DS-157). This form is
required only for male applicants aged 16-45.
Your current and previous passports.
A 2 inches by 2 inches photograph on a white or off white background.
The photo must have been taken within the past 6 months and show
your full face.
The pink copy of the receipt from any RHB Bank branch for payment
of the application processing fee (RM380).
Any other documents you believe will support your visa application.
