Non-immigrant Visa Services (Until July 27 2007)

Requirements and Fees
Applicants must apply in person and bring:
A completed Electronic Visa Application Form (EVAF) DS-156.
This form can be accessed using the following United States Government website:
https://evisaforms.state.gov
This form consists of three pages and all three pages must be printed out and submitted at the time application for visa is made.
Note: Men between 16-45 years of age must also submit Supplemental Nonimmigrant Visa Application Form DS-157. This form can be obtained at the Embassy and filled out by hand on the day of application.
A passport valid for a minimum of six months beyond the anticipated length of stay in the United States. The passport should be good to travel to all countries including the United States.
A recent 2" x 2" passport type photo. Each applicant regardless of age requires a photograph.
Important Note
Persons born in countries other than the United States may have a claim, under United States law, to United States citizenship if:
Either parent was born or naturalized in the United States.
Either parent was a United States citizen at the time of birth of the applicant.
With few exceptions, a person born in the United States has a claim to United States citizenship. Any applicant believing that he or she may have a claim to United States citizenship should not apply for a visa until his or her citizenship has been determined. The applicant should inform the consular office immediately of a citizenship claim so that steps can be taken to determine if the applicant is entitled to a U.S. passport rather than an immigrant visa.
